A thin old layer lying flat between the joists
The photos show old insulation that had thinned out to well below the tops of the joists, grey and yellowed across the attic floor.
A layer that thin does little for the rooms below, and adding more on top would have buried the gaps along the framing. Taking it out gave the crew bare plywood to clean, sanitize and seal first.

Seal and rodent-proof
Entry points closed with wire mesh, polyurethane foam and cement, with two to four indicator traps left as a check. Gaps around fixtures, piping, wiring and vents air-sealed in the same pass.
New insulation
New R-38 16in Fiberglass Batts installed across 892 sq ft, on a clean, sealed surface rather than on top of the old layer.

Floor protection and dust barriers were set at the access point first. The old insulation came out with a heavy-duty vacuum, rodent waste, droppings and nests were vacuumed out, and OdoBan was applied to neutralize odors, pheromones and bacteria. Openings were sealed with wire mesh, polyurethane foam and cement, gaps around fixtures, piping, wiring and vents were air-sealed, and two to four indicator traps were left as a check. New R-38 16in fiberglass batts went in last, across all 892 sq ft.
